# Official Reports and The 'Black Boxes'

* The **Preliminary Report** will likely be the first summary of all the factual elements of the flight that we see, typically released within **30 days** of the accident
* Both of the black boxes are reported to have been recovered. It's reported that while many countries often send theirs to France or the US for reading, India now has their own laboratory with said capabilities, so they may be staying in-country

The 'two recovered boxes' are likely just both EAFRs.



# Unofficial Reports

* A number of 'Preliminary Reports' have been circulating some media sites, including one stating the pilot's seat mechanism failed and he slid back and grabbed the throttles, complete with transcripts from a CVR that hasn't been opened yet (16JUN). Exercise critical thinking and appropriate suspicion of any such reports until they come from the AAIB or the NTSB
* Reports of the crew's Mayday call consisting of anything other than 'Mayday' so far appear to be false, at least one apparent source of the extended 'losing power' call has since retracted their claim



# Elements of the Flight

* It appears with fair certainty the **Ram Air Turbine (RAT)** was deployed as it passed by the home of the teenager who caught the crash on video. There is no apparent sound of takeoff thrust accompanying the propeller-like noise of the RAT, which supplies minimal hydraulic and electrical power to the aircraft.
* Reports that the **flaps** were inadvertently raised appear to be false. Leading edge slat deployment appears to be visible. There is no system on board the 787 which prevents retraction of the flaps against the pilot's 'requested' position, only physical 'gates' to guard against inadvertent movement beyond certain positions \[see Tech Notes below\]
* It appears retraction of the landing gear may have been initiated as the 'bogies' or 'trucks' appear (in bystander video) to be tilted forward, the first step of retraction
* Initial claims the aircraft took off with less than half the runway appears to be false based on incomplete flight tracking data, the aircraft backtracked to the start of the runway for a full takeoff run. Likewise claims it used excessive amounts of runway or kicked up additional dust than usual appear to be unfounded.
* A lack of rudder input in the CCTV video of the takeoff appears to rule out an initial single-engine failure. That video also appears to rule out any visible evidence of a catastrophic bird strike


# Other Elements of Discussion/Concern/Query

* An ANA 787 had an unexpected dual engine shutdown after landing on January 17, 2019, activated by a system called the TCMA (Thrust Control Malfunction Accommodation), intended to shutdown runaway engines on the ground. Its logic should only activate it on the ground with weight on wheels if it senses the thrust lever is at idle but the engine is not
* The Airworthiness Directives relating to the potential loss of all AC power due to an aircraft being powered on for 248+ days (integer overflow causing GCU failsafe) should have been rectified under AD 2018-20-15 by the end of November of 2019. For those not in the know, Service Bulletins are optional. Airworthiness Directives are **mandatory**


# Technical Notes

* Beyond manual deployment, the automatic deployment conditions of the Ram Air Turbine (RAT) include:
   * both engines have failed
   * all three hydraulic system pressures are low
   * loss of all electrical power to captain’s and first officer’s flight instruments
   * loss of all four EMPs (Electric Motor Pump, Hydraulic System) and faults in the flight control system occur on approach
   * loss of all four EMPs and an engine fails on takeoff or landing
* Circling back to the flap handle gates, [this video](https://www.youtube.com/shorts/q9voNK5Ss9k) shows them in action (though on the 737 which has different flap positions, the physical logic is the same)

* There are three separate hydraulic systems, Left, Center and Right. The Left and Right can be powered by Engine-Driven Pumps or Electric Motor Pumps. The Center is only driven by EMPs or the RAT
* Regarding Flight Envelope Protection (relating to the flap comments earlier), again, Boeing designs their systems different than Airbus and lets the pilot typically have the final say. The FCOM multiple times makes clear when speaking on these protection systems that they "do not reduce pilot control authority"

But they're still waiting for the black boxes to get hard info.

\- **The only survivor’s account in a more recent video (NDTV)**; He mentions that 5-10 seconds after liftoff that the plane seemed to be ‘stuck’ \[I think that is referring to the obvious deceleration as seen in the CCTV video which would be fully explained by a significant loss of thrust\]. Then he said that a bit later, ‘green and white’ lights came on \[if correct, this would likely be the emergency lighting system, especially as he was sat at the emergency exit row with the signs close to him\]. This fully tracks with a dual engine failure \[the emergency lighting which would be armed at that stage of flight. would automatically switch if you lose the normal electrical system\]. In this interview he does not mention the loud bang as reported earlier. The poor man is obviously in shock and I wish the media would give him some space.  

\- **The distinctive sound of the RAT**. There is a noise at the start of video 1 (on the versions with the original noise), which does not correspond to engine sound. This is almost certainly the RAT, based on another video of a 787 flying past with the RAT deployed. Based on the trigger conditions of the RAT, one or both engines and the electrical system would not have been working. 

\- **The landing gear retraction** (not considering the drag aspect, but the ability to even retract the gear). I think for a split second you can see the main gear starts to retract but then it stops, this is around the time that there is no longer positive climb. This would make sense in case of a dual engine failure and the switch to emergency systems means only a gravity gear extension would be possible (but no hydraulic power to actuate LG doors and retract the gear itself). As many have pointed out, the tilt of the gear is more evidence of interruption of the retraction sequence. 

The reports of what the pilot communication with ATC was exactly, I’m not convinced is from an accurate source. But the Mayday call alone as I said before, shows the crew were aware of a desperate situation on board. And in case of a dual engine failure, they wouldn’t have had the chance to do much at that stage.

One part I’m still wondering is the claims of the landing gear being tilted due to a gear up command that did not complete. 

If the gear had started to ascend and then the plane lost central hydraulic power due to damage to the electrically powered central hydraulic system it seems like that could explain the sequence of events we’ve seen. 

If the gear were tilted for another reason then it wouldn’t mean much. I don’t think gear staying down means anything in this instance obviously the crew had bigger problems.

The only other remote possibility would be a sequence of commands from the cockpit that are so egregiously bad they would almost have to be malicious.

I see a lot of different components, electrical systems, data recorders etc have independent and redundant power supplies. I'm just wondering how that is done? Cause I imagine if you go follow the chain high enough to the power source if there was a massive failure it could take a whole lot of systems out?

Posted this in megathread 2 but still baffled. Has anyone seen any drone shots of the wreckage from above yet? I've only seen various angles from ground and building.. Hard to get an idea of how everything landed. Did the front of the fuselage end up at ground level even though the plane crashed into the 6th floor?

Was he riding this thing till it stopped and jumped out? (sounds like the case) vs. catapulted out while it impacted. It would be interesting to see the crash site from above to get a better idea of where all the pieces landed..

1. AI171 accelerates with take-off thrust set
2. Just after V 1 the throttle-lever sensor fails and drops to 0 V, so the computer thinks the levers are at idle.
3. ~0.3s later, the TCMA sees that:
- Allowed-power line has slid downward far enough that the engines (still near full thrust) is above it.
- The aircraft is in fact still on the ground as it has not rotated
4. All conditions are met and TCMA shuts off fuel to both engines.
5. Even if the pilots are getting warnings, they are passed v1 and still committed to continue.
6. AI171 lifts off while running on fumes, and after a few seconds both engines flame out.

The plane looked totally healthy all the way up to collision with ground structures. The engines just… turned off at some point. Excluding malfeasance, if this is not a software bug, it’ll be some crazy Swiss cheese hole alignment. If this is a software bug…

I think it is now quite certain that both engines failed. The only question that arises is what could have caused this? Bird strike or fuel contamination can be ruled out as A bird strike could would be clearly visible in the video and fuel contamination would have atleast slightly affected all other flights departing from Ahmedabad as well. Could it be that all magnetos/ignitors failed? Requesting all possible theories

What happened: In Airbus A340 engines, failure of the Permanent Magnet Alternator (PMA) led to ECU power loss. Due to a software defect, the FADEC ECU did not switch to aircraft electrical power and shut down the engine in-flight  .

I just got off a flight on a 787-8…these big crashes always affect me for a few days. I wasn’t scared or anything just at takeoff was thinking of those poor souls. Can’t get my mind off it especially during a flight/travel day. Life is so unpredictable. I like traveling but used to be an anxious flyer (now I’m fine) and it’s so strange how we know that aviation in general is very safe but there’s always a feeling in me that somehow a big plane like 787, a350, 777 etc just feel safer than say a320…you get a feeling of comfort and amazement at these big planes being able to fly, and being in the same plane model as this tragic crash was just eerie. Such powerful machines to think of such a tragic event happening and here we are.

Just to add a different angle. The RAT is not ONLY deployed in engine failure. It can also be deployed in extreme cases of hydraulic loss, also it can be deployed manually

Given all I've been able to gather, here is my theory for what may have happened, keep in mind, it's all very speculative:

What we know:
•The plane only got up to about 200ft AGL
•The RAT was deployed
•The emergency lights most likely turned on
•The engines probably went into idle, or just straight up failed
•The flaps most likely stayed down
•The landing gear was never retracted

What do I think could've happened:
As soon as the aircraft became airborne it experience a total failiure of the AC buses, and for some reason the battery wasn't on, now, it is really unlikely, however, I think it fits, and this is how it goes:

Around 150 AGL the failiure happens, immediately 2 things happen, first, the RAT is deployed, as well as the emergency lights turn on, but also the engines's FADEC's lose contact with the cockpit, and failsafe either to a low power setting, or they failsafe into idle, of course this takes a couple of seconds so the plane climbs to about 210 AGL before finally topping out and starting its descent, the rest of the flight featiring the pilots looking into where to put the bird down.

Why this doesn't completely fit:
•The captain made a mayday call after whatever happened, happened, so the radio was somehow operational
•If the RAT had provided enough electrical power for the radio, it probably would've provided enough for the engines

I just don't understand how the birds strike and shutting down wrong engine theory have any grounds.

1. You don't see any explosion or smoke on CCTV footage. Not like Jeju Air crash, where both hits could be clearly seen from éven longer distances.

2. From rotating until the start of fall it took like 10 seconds. I don't think that it is enough time to notice 1 engine failure then decide to turn it off (wrongly the other). And we are not even counting the checklist to do that. Simply impossible.
And top of that there is no visual evidence that one the engine was off AND then other turned off.

While I understand that the final report might take months if not years to be released, if there was an issue with the dreamliner, I hope that they will release that information ASAP

Press Briefing from Aviation Minister: 

- 8 of the 34 Air India 787s have been inspected; nothing has been found. Again they’re not looking for anything specific just looking for anything that may look wrong in the engine or fuel systems etc etc. 

- The report should be out in 3 months

Can I say that any of the issues that have been raised by Boeing staff in factories point to things that may lead to structural failure, none of which have been corroborated. Now I’m sure that given what we know were there people doing things the way they shouldn’t have been, absolutely. Does that mean the aircraft isn’t safe, well the Dreamliners safety record speaks for itself. 

Now relating to the air India crash, as the video shows the whole plane was in tact up until it crashed so there obviously wasn’t a structural failure. So anything about final assembly is most likely irrelevant at this point

I’m not sure if other people can attest to this but it just feels so much more real, those pilots probably had the same dreams and career goals I currently have. There's way too much of this, not just here but by professional pilots who are acting as paid Boeing shills and spouting nonsense. 

And then repeating debunked theories like 'flaps weren't extended' or 'pilot shut off the engine!', 'pilot didnt retract landing gear !!' all designed to shift blame to the pilots.

This took 30s. There is simply no way the pilots, even if they wanted to, had time to do any of that. At that point in flight, literally nothing else matters except gaining altitude - this is repeatedly drilled. Any further troubleshooting is done only when you have some altitude and thus breathing space.

And yes, planes do have mehcanical and systems defects that can cause this. This is literally how the process works and makes aviation safer - something fails, sometimes it results in tragedy, investigators find out what failed, and fix it. sometimes the cause is human factors, which then results in better training procedures. Usually when its pilot error like AF447, it needs some time to develop.

Every single manufacturer repeatedely tests and certifies all systems on a plane to make sure this won't happen, and it still does. No one thought the 737 MCAS would cause a crash. It took 6+ months to find out BA38 had loss of thrust due to defective design of fuel inlet tubes. It happens all the time for seemingly perfect designs.

Well, when I say every manufacturer, I mean everyone besides Boeing. Lets be clear, they would love to find any cause that doesn't come back on them, and its not like they don't have a history of evil practices.

The airplane's Ram Air Turbine (RAT) deployed, which would indicate a major system malfunction.  There was also a reported transmission of the captain saying the plane had lost power and had no engine thrust.

A working pilot came up with the interesting theory that one or some of the lithium batteries malfunctioned and cut power to the engines- faulty lithium batteries have been an issue in 787's before. EDIT - one survivor identified and is currently undergoing treatment at the hospital, and the doctors are hopeful that the survivor will pull through.

It was a LOT less than 5 minutes after takeoff, more like 20 seconds. Crash site is about 1.5 km from the end of the runway.

Looks like dual engine failure. Pilot made mayday called. Ram air turbine can be seen deployed. Plane didn’t stalled but pilot flew it all the way to the last second… RIP.

This is either maintenance issue or possible defects with the plane. Note that several Boeing ex-engineers did came out and raise concern with 787 quality control issues before… so take that with a grain of salt.

Btw, they are ex-engineers because they were fired.
